Appointing a new Executor to a will

Could anyone tell me what the normal procedure is for appointing a new executor to a will

In a scenario, for example, where the original executor took out a grant of probate after the death of the testator, but then died before dealing fully with the estate ?

In this scenario there are three beneficiaries named in the will. One has a right of use, the other two are residuary devisees and legatees
