Yikes! Load falls from Crane

So, I had to go to a few places up in Dublin today

It was windy mid morning in the Northern Cross area. In the apartment site opposite 'Parkside' I noticed a tower crane with a load at high level. Load was clearly catching the wind judging by how it swayed back and forth. Then it came loose and the individual items of the load fell.

With such emphasis on safety these days how could this happen ?

( Mods please move if inappropriate forum - thanks )
