Had a look at the NDLS site recently?  To change your license address you need the completed application forms, medical and eyesight reports.  Is this overkill for an address change?

Does your address match that on your driving license? 19 votes

    Where you getting this from?

You only need medical reports if your medical details are changing.

    Right here - https://www.ndls.ie/how-to-apply.html#change-of-address
If your address has changed in any way since your last application you must submit:
[*]Application form for Driving Licence D401 (Fully completed)
[*]Current/ most recent driving licence / Learner permit
[*]If you do not have your most recent licence you must present a completed lost licence declaration (question 17 on D401)
[*]Evidence if your new address from the approved list and dated within 6 months
[*]NDLS medical form (dated within 1 month) if required
      [*]Evidence of IBT if required
      [*]Evidence of CPC if required
      [*]Evidence of PPSN
      [*]Relevant fee

      if required.
If you don't need a medical form to drive all you need is the application form completed, current license, proof of new address and evidence of PPS. No fee is required either.
