Water from hot tap changes from hot to cold

Hi All the hot taps in my house switching between hot water to cold water and to hot water including showers. I have a condensed combi boiler and a pump for water tank downstairs. Any ideas what cause this problem?

May be an issue with water flow rate from the pump, too high or too low or a problem with the hot water heat exchanger.
Is the water pressure dropping and rising slightly as the hot tap runs?
