Renewing learner permit problem

Hi, I want to renew my learner permit as it expired on 6/4/17 but I lost it a while before that. Should I apply for a normal renewal or a lost/stolen permit (since I did in fact lose it, but it is now out of date so it won't be of any use but I can't show them the old one because I don't have it anymore)
Also, do I need to get an eye test again or is the one from my first application okay ?

    Thanks.

It'll be a renewal but you will have to declare it lost by way of getting a Guard to stamp an sign your D201 application in the relevant spot. As your old permit would have been your ID, you will now need to bring photo ID such as passport or the photographic PPSN card but bear in mind that whatever way your name appears on your ID will be the way it will appear on your new permit.

You won't need an eyesight report again (unless you used to need glasses and no longer need to.
