Declaration of unemployment card

I got a small green card today asking was I unemployed from the 16 of march to the 12 of April. Says to return it to the local office as soon as I have it completed. Do I have fill it now or wait till the 12th before I send it back. New to all this stuff. Thanks

If this is for 2017 then u can't answer it until the 12th April very strange though
