Primary Medical Certificate - Parkinson's Disease

A family member has Parkinson's disease. She is mobile, does not need a wheelchair however has very reduced mobility. Walking takes a long time (shuffles along) and the tremors are affecting her ability to complete tasks with her hands. Is she eligible for the primary medical cert and if so, would she also be eligible for a VAT exemption on a car? She has a small car and we thought that a higher vehicle, like a qasqai or a tucson would be easier for her to get out of. I'm not sure what adaptations would be needed, maybe a swivel chair..
What would you all say her chances are? Obviously parksinson's is progressive...
