Building a questionnaire and taking data from questionnaire to produce a report

I am wondering does anyone know any free applications or even better any of the Microsoft applications that I can create a survey and then depending on the results populate a report.

So if I had a generic report "My name is ______ , I go to school in ____ , I have a _____ as a pet.

If my questionnaire had three questions
1. What is your name?
2. What province do you live in (This can fill in a province for where I go school) This can be a multiple choice question.
3. A question with a checkboxes that lists a load of pets and you can tick which you have.

If anyone knows anyway I can do this I'd appreciate it.

Might work. Other than that Google Forms are quite good too.

Google will automatically create a report for you too.

Google forms is close but not exactly what I want I think. If I asked a multiple choice question of have you a dog with a yes or no answer and they choose yes I want to be able say they have a dog not Yes which is all I can seem to do when I link excel to word.

Any help appreciated.
