I got my Public Services Card this morning which has my free travel pass printed on it. However my card only has the free travel for me. Because of my disability I'm not able to travel by myself. Is there a process or form to complete to try change the pass to allow free travel for me and the person traveling with me.

    Are you under 70 OP?
Here's the form for you to print off and complete.
YourGP has to complete part of it.
I'm afraid there's quite a backlog so get it done ASAP to get in the queue

The GP doesn't need to fill it. Just write " rhuemetism or arthritis " yourself.

Any idea how long the back up is? Sent my form off about a month ago. Do most applications get approved?

Once you send the under 70 form off it will eventually be forwarded to Dublin for the attention of the Medical Officer.
The MO is not available all the time and has a lot of applications to get through and Companion passes are not a priority.
If the MO does approve it it will be sent back to Sligo to be processed.
You are looking at at least 10 weeks if not more.
Under 70 is pretty hard to get depending on the evidence presented
