My wife changed jobs and was put on emergency tax for a month.

Has now got her tax cert.

How is the balance made up. Is it applied over the year via tax credits or should she get a balancing statement and a cheque?

She'll get it back in her salary payment once her P45 has been processed by Revenue and her new employer.

In a one off payment or spread over the year?

Depends when she was on emergency tax. If it was in 2016, apply for a P21 balancing statement online and they'll refund you (cheque or bank transfer).

If she was on emergency tax this year, she'll get it back through her wages if on a cumulative basis. Depending on amounts involved, might just see it as reduced tax when paid next.

I've always received as a once off amount on top of normal payment.

It was this year. approx 50% of her salary

I'd expect to see it in her wages then in one payment. Might not be the next payday, depends when the employer does their payroll update so watch for that.
