Help with filling in dole form

    Could anyone help me with a dole form that I got to fill in as I'm unsure of a certain part of it.
    I work 3 days a week, Monday, Tuesday & Wednesday (the same 3 days every week). I have to fill out the hours I work from Monday to Sunday over the last few weeks.
    I work 21 hours every week, but for some weeks when I'm due holidays, I'd only work 2 days that week but I would still get paid for the 3 days. So when it comes to the form for that week, do I still put 21 hours as this is what I'm paid, or would they expect me to only write 14 hours - the actual hours I worked. I just don't know what to fill in.
