copy of speeding fine nomination form?

    Hello,
    Can anybody shed some light on this for me please? I am looking to get a copy of the nomination form I filled out and sent back to the fixed charge processing office nominating me as the offending driver on a speeding ticket. I very stupidly have mislaid the copy I took and want to ensure I put the right address on it?
    After my husband received a letter to confirm the nomination I never heard anything until yesterday when a summons was put in my letterbox but with the wrong address on it. It was addressed to a rented house on my road which has a high turn over of tenants and I really want to make sure I put the right address on the nomination form before going to court so I can tell the judge I did not receive anything in my name until the summons.
    Is it possible to obtain a copy of this document? if so who should I apply to? the summons is issued by Dublin Metropolitan District Court by the hearing is on in the Criminal Court??? The hearing is not for another 5 weeks.
    HELP PLEASE

    I tried this morning they do not have a copy of it, the original goes to the courts and the judge has it at the hearing!! so am panicking as to whether I filled the address in wrong. not something I would normally do but am just worried i look like i:eek:'m trying to pull the wool over their eyes and it would be a genuine mistake.

    If you end up going to court you can explain what happened to the judge on the day,as long as you aren't trying to get away with it and want to sort it out he will be accepting of your situation

    Yes, I live in 28, summons addresses to 24 which is rented so obviously the tenants didn't know me but somehow the summons made it to my letterbox yesterday??????

    It's a mystery.... Rented house tenants don't speak sword of English!!! So can't even ask them... :eek:
