    I completed my Form 11 on Revenue and paid the relevant tax. My bank is looking for the Chapter 4. I have downloaded the completed Form 11 from Boards but have no idea what/where is the chapter 4?!

    It's the modern version of a notice of assessment that you get back from Revenue.

    I found it thank you.

    In case anyone else is looking for it it was ROS -> Revenue Record -> Inbox I searched by date. Couldn't find it using the other filters.
