    yes, in a hospital its the clinical director and the CEO

    As a GP I was self employed for a long time and effectively my own boss.
    Now working for a large company and have a line manager and boss above that.
    It all depends on circumstances.
    All docs in hospitals (public) have bosses.
    In private hospitals it varied but most don't and are self employed.
