Social workers may spend a lot of time in the office writing reports etc. Social care workers would usually be involved in providing direct care for a vulnerable person. Sometimes people who qualify as social care workers go on to do a masters to qualify in social work but I don't know anyone doing it the other way round however sometimes there are jobs such as Project Worker jobs that require either qualification.
