Questions on the forms for divorce.

    We're getting a divorce... YAY
    Both of us are amicable and we have the access all set out between us. Maintenance has been paid every single week since he moved out. We want to do this on the cheap and I am good with forms... I thought lol


    I am using the forms and just filling in relevant info.

    I have not a clue on the 2N form it says:
    AND THE APPLICANT CLAIMS:
    (Here provide a numbered list of the individual reliefs sought, by reference to sections of Act)


    What do I put there? Been searching around I cannot find any example :(

    For the Affidavit of Means is there an example somewhere of how to right the whole scheduling thingy?

    Last thing, for the "CERTIFICATE PURSUANT TO SECTION 6 OF THE
    FAMILY LAW (DIVORCE) ACT 1996"
    How on earth do I swear that I told myself about reconciliation... It is written as if the solicitor is saying it but I don't see a point when I can get a commissioner of the oaths right?
