Did you miss this bit ?

"Giving medical cards to every prisoner on release day would not cost the State, he said, because a lot of inmates were on a level of income that entitled them to a medical card anyway"

No, I didn't miss that bit. The medical card should be given to children and adults who have a genuine need for it. Not to be issued to some junkie for his methadone.
