I lost my licence..just about to apply for a new replacement licence! I was wondering do I need a medical report as it says on the website, I'm young and only got my licence 18months ago and have no health issues. I presume an application signed by the gardai is enough

Should be the same procedure as getting the license renewed. The instructions will be on the application form.

If nothing has changed as regards your personal details then all you'll need to do is fill out the app form, have Garda fill out the relevant section and stamp the form to allow you to declare your current licence lost. Bring valid photo ID such as a passport and €35 to the NDLS centre and you'll get a reissue of your previous licence.
