Insurance for more unusual locations?

Can anyone recommend a travel insurance provider for more unusual travel locations? The cover has to have full medical as a private policy with Aviva will not provide medical cover. I've read reviews for WorldNomad and they aren't favourable. Anyone suggest another company?
