Hi, I have recently within the last few months been denied a medical card and instead been giving a GP Visit card, my circumstances have since changed and I feel I may be entitled to my Medical Card back but how do I go about re applying for it or do I have to wait for the HSE to send me a review form? Any help would be…

Contact the HSE and inform then that your circumstances have changed.
