One parent tax credit revenue issue

    just looking for some advise

    just a summary of my problem:
    I claimed single parent tax relief for 2009+2010+2011+2012 as we had a baby in october 2009 .
    we did live together for a few months early 2009 but we broke up and i moved out.
    i own a house which i moved into in 2009 until early 2011. i then rented this out for 2011 + 2012
    (filed tax returns etc) and i moved in with my mother as my father died in 2011.
    we (mother of our child and me) started meeting again and ended up have another baby in 2014 but we didnt live together until we moved in together last year.
    i received a letter from revenue in 2014 saying in their opinion i was living with the mother of my child for 2009/2010/2011/2012 and asking both for proof etc.
    My partner sent them esb bills/gas bills/sky/phone/house insurance/bank statements etc to show them she lived on her own and paid all bills etc.
    i sent them bank statements/car/house insurance/car insurance/esb/gas and everything i had to show my proof of where i lived.
    they said i made a dentist tax refund with the address of where my partner lived, i explained when i lived with her in 2009
    i had to go to a dentist and gave them that address and never changed it, and in 2012/2013 when i made the return the dentist
    filled out the form for me and they obviously used my previous address which i didnt see as i just signed it.
    i contacted them numerous times since then to confirm what we sent was ok etc, never got a reply.
    now 2 years later i got another letter saying i have to pay back single parent tax relief for 2009/2010/2011/2012
    but i have sent them all the proof
