QR code scanner to open preset URL with QR value

    Anyone know of a QR code scanner app for either iPhone or Android that allows you to open a URL with a parameter passed from the QR code.

    I am creating QR codes with a unique code (e.g 743289) but I would like the QR scanner to append that code to a URL and then open the browser.

    QR Value = 743289
    Redirected to http://www.myurl.com/743289

    I do not want the users with the QR code to know the URL which is being opened.
