PSDP Appointment

Under the Construction Regulations 2013 and earlier the Client must appoint a competent etc. PSDP and .....etc. Also the PSDP must accept the role.
Are there any statutory forms for each of those i.e. Client appointing and PSDP accepting. I cant find anything on the HSA website and the closest is Form AF1 for notifying them giving details of PSDP among other things.
That being so has anyone seen or used standard forms/styles used for such appointments. I know what I need to do but no point in reinventing .....etc.

We almost always just use a letter from the client and then write a response on our headed notepaper but I have a vague recollection of seeing an overly complex form on a project last year. Not on the computer now but will check later/tomorrow and let you know what I find.

Scope of works is important - make sure that things can't be misread to define you as PSDP for some other part of the works that is outside your remit.

Thanks MT for that. Great help here, as always - just wanted to make sure I don't start on wrong side of HSA. Perhaps keeping it simple is the best.

Have a look at Model Form (MF) 1.10 at the attached link PMBC:

    http://constructionprocurement.gov.ie/model-forms/

I knew I saw it somewhere.

Like I said - I've seen it used twice ever I think - both times by the same client.

Yea, just opened that and read. That would be 'the ultimate' so Ill use as a template, slim it down etc. The handy thing is its both appointment and acceptance.
    Thanks again:eek:
