If you are talking about putting a new door in the outer (main structural) wall of a two story house, don't let anyone other than a professional builder near it. I would suggest getting a structural engineer or similar to advise.

This assumes its a garage extension
    the title says
Create door in house wall into extension
    the body says
    with garage extension

Doors directly to/from garages are not that common in these parts, one of the issues is that the door has to be fire rated as its accessing a garage.

It will require a fire door which is the whole lot, door, frames, hinges, locks and the fitting so clearly not a Saturday morning project for a man with a van, a kango and a can of spray foam.

It is not my area of professional expertise but the following pointers are pertinent.

    It is dealt with here
    http://www.environ.ie/sites/default/files/migrated-files/en/Publications/DevelopmentandHousing/BuildingStandards/FileDownLoad%2C1640%2Cen.pdf
    which is
Part B of the TGDs from here
    http://www.environ.ie/housing/building-standards/tgd-part-d-materials-and-workmanship/technical-guidance-documents

All fire doors need to be self closing so that in itself can be a nuisance.

There are also fume egress considerations..

What ever you do, you will need full professional sign off as doing it wrong and you have a fire, your house insurance wont pay.
