Entrance onto road already exists, is PP required to put a roadway from entrance down field along the ditch?

My understanding is that this wont require planning provided you are not intensifying the use of the entrance and that it remains as agricultural use only. However i would urge you to speak to the local planning authority who will be able to offer an authoritive opinion - you can ask the question hypothetically if you dont want to draw attention to yourself!!!:D

There is an exemption for the repair or improvement of a private road but I am unaware of any exemption for the construction of the road.

If laying out a roadway with say hardcore then planning probably needed. If just clearing the ground for a dirt track then planning probably not needed. Assuming it's not its some conservation area or archaeological site or something major that you would know about already. Talk to your local authority.
