Disability Allowance Applcation

    My GP suggested that I put in an application. I have picked up the form (book!) but obviously there is a lot of very personal ie financial information that has to be entered.

    This form then has to be given to my doctor to complete, and then back to me to send it off.

    Can I give it to him before filling it all in? And then complete it? I don't really know why he should be privy to all my personal financial details, eg my accounts and how much savings I have!

    Or am I mistaken and have to complete it and then he sends it off??

    The doc won't give a toss about whats on it, Al s/he'll be looking for is the page that has to be signed. And you can be sure that the doc won't be signing his/her first form. S/he will know that form backwards most probably, and will know the appropriate page to open. In any case, they are bound by client confidentiality, so no need to worry.

    Ah I know but.....it doesn't need to just be signed, there is a whole rather long section to be completed by the doctor. I just wondered if I could give it to them not filled in by me, and fill in the details myself later.

    Absolutely fine dragona
    Just sign and date the page where you give permission to the doc to complete his part and leave it in to him
    You can complete the rest when he gives it back to you
    You post it yourself

    Didn't want to post a new thread so.....doctor has filled out his part, now I am stuck - do I apply and list my daughter as a dependant? She is 19, not in college yet but will be in September,she is currently getting €100 a week Jobseekers, her grant has been approved already., just waiting on the CAO offers.

    So not CURRENTLY in full time education, but will be.

    No she's not a dependent when she's getting her own payment.

    Well thats the thing....she is not a dependant now, but will be in September when she starts uni......

    I know what you mean but if you're putting in a claim now you put in how things are at the moment. If she was in college and on holiday you wouldn't get the extra while she has her own payment.

    Even then you can only claim if she lives with you.
