Nursing home reviews: who should fill them out?

    Hey all,

    I just have a question if anyone can help regarding the questionnaires nursing homes use during a review with family/close ones about a patient's care.

    So far the nursing home have told me the reviews were for their own records but it turns out, that is misleading - they are supposed to be sent on to HIQA and signed by both nursing home and family members, so it isn't purely for the nursing home only.

    Is it normal for the nursing home to ask questions and fill in the answers you give, or should families be asked to fill them out themselves? And perhaps even offer to post to HIQA for better freedom to say what they feel without fear of repercussion?
