Leaving Cert Subject Form

    I was wondering if there is a form that you fill out in sixth for the SEC that says what subjects you're doing and what level?
    And if there is such a form , around what date is it usually filled out?Also is there any way you add a subject after that date?

    Usually gets to schools Feb or so. You can't add subjects after that where there's a practical or oral element.

    You can change levels for LC on the day of the exam.

    Thanks so much.So would it be possible to add a subject like applied maths 2-3 weeks before the exam?

    Yes it should be. There's no issue at all if it is already a subject taken in your school and you show up on the day. If it's not already taken in the school, they need sometime to organise a centre and supervisor, but before the middle of May or so should be OK.
