A friend of mine has a medion MD8341 and the drive is almost full. Could a new drive be fitted to have two hard drives in the case? Is this possible with any PC or would it depend on the motherboard

Depends on the motherboard and the case, at the very least he can replace the drive altogether.

No sure the spec sheets give that level of detail, can you open the PC up?

Only way to know would be to open up the side of the case and look at the hard drive rack. There is usually space for about 3-4 hard drives so I would assume it would fit no issue. All you have to check for is a spare sata port (looks like a a L shape connector on the motherboard) and a spare sata power socket (longer L shape coming from the power supply).
If both of those are there, pick up a Western Digitial Blue 1TB Hard drive, or something similar.

Edit: This is assuming the PC isn't ancient and using IDE connectors.
