I currently have a Grasslin Ecosave timer as per the attached photo.
The hot water tank is electric.

Is it possible to replace this with a heatmiser-hw or similar thermostat?

Heatmiser is primarily designed for central heating systems being linked into peripherals with low amp ratings of approx 3 - 5 amps.
Immersion elements have a much higher rating of approx 12 - 13 amp, probably not a good idea to mix the two.
