Immigration - forms needed for GNIB card renewal?

    Hi, I have been a student in Ireland for the last 4 years, and have stayed on to work here. I now have a (graduate) work permit, but understand that I still need to renew my GNIB card.

    This is the first time I'm doing this as a non-student - what forms do I need? Do I still need a bank statement proving I've got >3,000 euro, or not?

    Here's what I think I need:
    -passport
    -old GNIB card
    -work permit
    -proof of address
    -proof of health insurance

    Anything else?? I can't take the time off work to go more than once!
