    I just watched the second episode of season five of Tim Allen's Last Man Standing. At the end Allen is talking with the Kyle character played by Christoph Sanders, and calls him by his real name (Chris)

    It occurred to me that I've never seen this happening before. I guess it's because it's usually picked up by someone on the crew or in post production. It wouldn't have been difficult to voice over the mistake, but to have it go to air seems really sloppy.

    I thought this would make a good thread. Has anyone else seen anything as stupid as this get through?
