    Doctor not a nurse.
You only need a medical if you have any of the conditions listed on the application form. If you need an eyesight report must be done by either a medical doctor or an optometrist.

Proof of address is only required if your address has changed or if you want to make a correction to the address on file.

Your old permit is your ID assuming you don't wish to add or change your name. If you're Patrick on your current permit, you can change to Paddy if you like as it's a derivative.

You won't need a medical report if you have answered no to all the questions. Note: read the report yourself carefully; it's very straight forward and really only asks your doctor to sign you off as healthy or not- make sure you fill out your own details properly and the doctor doesn't write their own name where they should write yours- this is a very common error

You will only need an eyesight report if you wish to remove the 01 notation relating to having to wear corrective lenses

Proof of driving test application is necessary if you wish to apply for a third or subsequent permit (ie if you have a 992 or 993 notation after the categories on your current permit; if you have a 991 after the categories, you're fine). If you need a test application, don't fret, you can apply tonight and bring the email confirmation you receive as proof- you don't need an actual test date yet. Having said that, if you applied for a test last year and postponed it, all you need is a valid test application on file so you may not have to apply again.

    Fee is €35 unless you're over 70
