How to make a study plan?

So.. 6th year has just begun and the pressure is already on! I have great plans to put my head down and study for the year but I have no idea where to begin! I've always studied haphazardously - some days for hours and some days not at all... This year this obviously needs to change! I'm desperate for help to make a study…

I'm desperate for help to make a study plan of some kind because I genuinely have no idea where to begin! Thanks in advance for any bit of advice at all you can give me!
