NDLS Eye sight report form

    Hi,
    looking to get learner permit,
    where do I get this eye sight form,
    (don't have a printer to print it out),
    where do I get this test done and do the people that do the test give me the form,
    Also where do I get the learner permit form apart from printing?

    You don't fill out the eye test form. The opticians that does the eye test, will give you the eye test form. It is not something that you can print out or fill out yourself. It has the results of your eye test on it, so only the optician who does the actual eye test can give it to you. That is the form that you then bring in with you to the NDLS centre, when you apply for your permit.

    Any optician can do the eye exam. Just tell them it's for the learner permit and make an appointment. If you're looking to save money, sometimes the likes of Specsavers will have a special price offer on, but any optician can do it.

    You can get the learner permit application form at the NDLS centre.

    What ProudDUB said. Also you can get the application form at your local library, local authority building, Garda station or motor tax office.

    Just got an eye sight test done for my C licence. Went to spec savers for an eye test and they said I'm short sighted!?!? Don't know how because my vision is perfect! They refused to sign the report until I purchased a pair of glasses....as I said to the girl in the store...."I could just buy the glasses and leave them in the glove box of my car and nobody would know?"

    Anyway I went and got a second opinion off another optician and he said I have perfect vision...form signed.

    In other words AVOID SPEC SAVERS!! They see people coming in to obtain a licence and they just try force a sale of glasses. I know a few people this has happened to.

    Hi rang spec savers to get the form and get my Local GP to do it and they sent me out a form but its for glasses don't see anything about driving licence on it.

    If I could just get the form,( you can download it on NDLS website) and get my GP to do it, will that work? thanks

    Not to hijack your thread, I completed my TT last week but seem to be getting conflicting info on where to get this done. The form itself specifies Medical Practitioner / Optometrist but can this also be done at an NDLS Centre?

    No. They don't do eye tests at the NDLS centres. There are no medical practitioners there. You can only get the eye exam done at an opticians/optometrist office, or at your GP's practice, is he has the equipment to do eye exams.

    If you haven't been called, emailed or written to about a discrepancy then I think it's time to call the NDLS and chase it up.
