New Medicare Entitlement Application

I'm just looking at this years form for medicare exemption and question 15 asks

Did you hold health or medical insurance in that country (e.g European Health Insurance Card)

It also asks for certified copies.

I'm pretty sure I had health insurance, might have been on my parents, but it was 4 years ago and I've no chance of have copies of anything.
What has anyone else put in for this?

My form doesn't have a question 15. What form are you working off?
