FIS Renewal

    Was wondering if anyone can help me please. I am currently waiting on my FIS renewal to be approved but have just received a letter today from department of Social Protection stating my employer has been sent a FIS 5B form to be completed & returned! Last year my hours per week were cut from 21 - 14 for a period of time & because it was only meant to be temporary I didn't notify the FIS department. My hours did eventually go back up to 19 but I wonder is this what the 5B form is about & will I lose my FIS entirely if they realise? Thanks.
