Medical card - expenses rate

Hi all,
I am in the process of preparing additional information in relation to a refusal of a medical card for a very serious long term illness.
Quick question - what would be an acceptable expenses rate for travel, parking, meals etc etc for a 100mile trip to and from Dublin (ie living 100miles from the hospital) for ongoing outpatient treatment?
Is it calculated at a standard rate per mile or should I propose a rate and they counter?

Look near the bottom of the page for civil service mileage rates these are near enough what the HSE will accept

Anything you can show them, I even had to scan bus tickets to work and thanks to that MC was granted for me, cause I was maybe 2 euro over their limits.
