Impact of Mental Health history on Occupational Health

    I would appreciate some advice or opinions. In 2014 I had a breakdown due to stress and anxiety and was diagnosed with depression, both my work life and personal life were completely out of control and I lost it, too put it mildly. I resigned from my job and took the time needed to recover. So fast forward to today, I am trying to find a new job and have been offered a position, all of which is great but.... I must now complete an occupational health form and I am very worried. Obviously I will tell the truth when completing this form but facts such as I took over 100 sick days last year, due to work related stress and anxiety and depression, and I resigned the position as yes I bullied and management refused to tackle the issue. I take responsibility for myself, I was very venerable at that time and my home life was also nightmare..
    My dilemma is do I complete this form or will I just let this position go and try to get something else. What happened is part of who I am now and which is grand but putting it down in cold hard facts on this form is has struck a cord with me. Mental Health is a very personal thing and I am not sure I want to share even the facts with anyone, and if I do what the impact of this may be? Has anyone any experience of Occupational health being impacted by their mental health?

    What is the point of not going for the position? Worst thing they can do is say no. But they probably won't.

    If you are now recovered and your doctor has said you are fit for work, then put brief information on the form - was ill with anxiety and depression. Now recovered. Fill in any requested info truthfully, but don't give your life story either. You don't have to go into reasons that are personal to you.

    Lots of people have suffered depression - as much as 25%.

    Filling in these forms usually comes after an offer - so they like you already. And the point usually is to see if they need to accommodate any conditions so your work does not damage your health, they are not looking for reasons to boot you out!

    Thanks for your reply. And I agree with you, for me I need to separate the personal issue and deal with the facts when completing the form. I just panicked a bit which comes from my confidence being knocked. Thanks again.
