Gum/Tooth Issue

    I am looking for some advice. I have periodontal disease which is moderate on the back molars. I've become very proactive in trying to keep it from getting worse i.e flossing every time after I've even eaten a snack; going to the hygienist every three months etc. I do smoke however I have cut down significantly and am going to quit shortly as I am aware of the detrimental effect it has on periodontal health.
    My main problems is I build up tartar very easily that builds up in places where I cannot floss properly.
    Also over the last few weeks I've been felling an uncomfortable heavy sensation over one of my molars. It feels like there is a heavy full sensation right above it. Bare in mind the dentin is exposed. Every day I wake up feeling this uncomfortable sensation of exposure on that tooth. It feels like the tooth is haning, or very exposed and almost separate from my mouth. I do suffer from sinus and currently recovering from it. The dentist says there is nothing wrong with the teeth. Any advice would be greatly appreciated as I would like to keep my teeth for another while.
