Employer won't send form explaining temp employment

    Whilst on JSA, I did a brief, two-day employment as a favour to a friend. I informed Social Welfare. However, I was told Revenue would still send me on a letter asking me to explain my payment of wages and the employer was to fill out a form detailing the nature of my employment. This happened before when I did a temporary job so no problem. I was also to supply my p45 and payslips with this form, both of which I have. 

    However, on this occasion, I received the letter, sent it on - and despite repeated inquiries, the employer has yet to send it back to me with their part of the form completed. My social welfare office gave me two weeks to return this letter (and they sent it a week late). This is up in three days. How do I approach my social welfare office about this?

    Just inform DSP that despite numerous requests you have not received the form back. Perhaps they have sent it directly to DSP. In any case this is between the employer and DSP now.
