It is the regular license application form. There isn't a separate one for lost licenses.

If your previous license was lost or stolen, you have to fill out Section 16 & get it signed and stamped by the Guards.

Thanks for that.Do you know by any chance if I can get it stamped in any station or does it have to be my local one?

I'd imagine any station would do. The folks in the NDLS centre have no way of knowing what/where your local station is.

hey do you need to take anything else to the station? Or is it just the form?

I'd imagine you'd be better of to bring some other form of ID just in case. If you're from the locality and or/ you're known there, you should be okay. Effectively, the Guard is witnessing your self declaration. You must bring ID to the NDLS anyway to avail of a lost/ stolen replacement licence.

However, if like the OP, the licence was issued in good faith and you simply didn't receive it because An Post lost it, all you have to do is call the NDLS, have them verify that the licence was dispatched and indeed, no error was made on their side (eg your name and address were issued up to date and correctly), they will simply issue you a new licence no questions asked, on the basis that if the original one shows up later, you will have to surrender that one back to them.

The replacement licence will have a code on it indicating that it is a duplicate of a previously issued licence.

(I had the same problem about 20 years ago. Licence was 'lost' in the post and I got a replacement. I recall having to write a letter of explanation for the Gardai at the time - may be different now. The original turned up a few weeks later. I'll have to admit that I then held on to both of them as it was handy to have an extra form of ID.)
