What's the eyesight test exactly that GPs do for a Driving Licence Medical Report?

    This Driving Licence Medical Report Form (no form number), is the 'fitness to drive' one that a GP completes one side of, one box of which is whether the applicant needs to wear corrective lenses while driving. (The other side is completed by the driver & signed in the GP's presence).

    I hope this is the right forum? I couldn't find one that GPs might frequent (but then I don't suppose they have the leisure to!).
