Job seekers to disability

    A relative, who was receiving JSA, has been taken very ill and has been in hospital. He is unlikely to be able to work for months.
    He's now missed 2 signing in days at the post office and has no money at all. What needs to be done to change his claim from JSA to Disability.
    I've been tasked with this and would be grateful for any advice

    A doctor needs to complete a SW Illness Benefit application form. These are available in your relations GP surgery . Explain to the GP receptionist what has happend and it can he sorted.
    Submit the completed form to the local SW office.
    Ask in the hospital for a " sick note" so to speak.
    Bring this sick note to your relations local CWO and explain.
    The CWO should be able to put a payment in place
