Not seeking legal advice, just curious how this would work. Sorry if it's a stupid question.

Suppose person A is doing something, such as cleaning his car, sweeping under the window sill, etc and person B approaches from behind an d deliberately frightens person A for fun.

If person A jumps and bangs his head would person B be guilty of an offence? (such as assault)

If person A reacted (fight or flight) and hit person B suddenly after person B scared person A, would person A be guilty of an offence such as assault?

If person A had a heart attack because of the actions of person B would person B be guilty of an offence of any other kind due to his actions directly (or indirectly) resulting in the death (or grave injury) of person A.
