If I were to get a B3 in ordinary Irish, and then got a D3 in Higher level maths, would Irish overtake maths as one of my top six, or is it done by points value?

It's done by points. Whichever subjects you get the highest points in are the subjects that count for the CAO, regardless of level.

If you needed to get a D3 in higher level maths but scored higher points in Irish , you would still get your course. For course requirements you don't need to have the requirement in your top 6, if that's what you are wondering.
