water charge application PIN help!

not sure if this is the right area but i got my water charge application in the mail and it asks for my PIN but i dont have one. it says it should be in the top right hand corner but the PÌN and APPLICATION NUMBER box is blank... I rang the number to ask them but was on hold for 100 yrs. Also on the water.ie site they say…

I rang the number to ask them but was on hold for 100 yrs. Also on the water.ie site they say to download a new form but again they come out blank. I havent received a letter about this before so never got a PIN. any advice would be appreciated thanks!
