    If I decide to leave my job after my maternity leave.

    Do I ask my employer " can I use your name as a reference "
    Or do they need permission.

    Or in my letter of resignation could I ask them for a written reference?

    What's the done thing nowa days

    You can just ask your employer can you use their name as a reference.

    A written reference can be useful, particularly in the scenario that a new potential employer is looking for a reference some time down the line, and nobody remains at your original company who knows or can remember you.

    At the risk of stating the obvious, make sure that the person who you name as the reference is going to give a good report on you if asked :-)
