Medical card documents request

    Hi I applied for a medical card. I received a letter in the post today asking for some documents. They are asking for a list of all shares and interests I have, letter from student grant and three recent bank statements.

    I have the last two but I have no shares or anything like that so I don't know what they are asking me this for, is this standard or just a mistake in their behalf?

    it's a standard request - just reply saying you do not hold shares or stocks in any company.

    hello,

    do you have to get the GP section of the MC1 form filled in after you fill in your details or would they have their section prefilled in forms in the surgery does anyone know?

    For a first time application you need to get the GP to complete it - for a renewal, no.

    You need to bring it to the GP and get them to sign it.
