    I there, I was wondering if anyone would be able to help me? I am looking for a copy of a UP15 form, if anyone could take a photo of the form for me to print out or even a link to it online?

    I've just gone through all the Forms on Welfare's Site but can't find that! That sounds like the Sheet D.S.P. randomly sends out to Jobseekers to fill in their most recent efforts to show that they've been looking for work lately and where they have applied for jobs to.... Or maybe that's UP16 I'm thinking of.....

    Not too sure either, I was told they send it out to you or give it too you at/after the interview.

    A UP15 is a form to be completed by an employer detailing the hours worked/rate of pay for the previous 13 weeks & is used by DSP to ascertain that a jobseeker has suffered a substantial loss of hours & pay in order to be eligible to claim.

    It's working for a temping agency would that make a difference ?
